If you qualify to submit into Santa Cruz via the CERTNA portal, please complete and execute the MOU as SENDER and mail the originals to CSC where we will complete and sign the MOU as AGENT. We will then mail the originals to Santa Cruz and notify you when you have been authorized to submit.
